Vendor/Exhibitor pre-registration, you must accept the terms
and conditions of the Omaha Dog Park Advocates Event

Rescue Booth.  - $150  Please submit 2 checks
$25 Payable To the City of Omaha
$125 Payable to The Omaha Dog Park Advocates.
In addition to my rescue booth I would like to include
materials in the goodie bags.
In addition to my rescue booth I am willing to provide the
following toys, treats, and or raffle donations.
